A damning report from the US Coast Guard says Oceangate, that owned and operated the Titan, failed to follow safety protocols.

The deep-sea catastrophe was the result of repeated failures by the company and its late CEO to follow safety, testing and maintenance protocols, the Coast Guard said.

The United States Coast Guard Marine Board of Investigation has released its final report on the company responsible for operating the Titan submersible, which imploded two years ago while attempting to dive to the wreckage of the Titanic, killing all five people on board.
